Checklist for Master's Thesis
This process ensures that the library has a copy of your thesis and that researchers worldwide can find your research through the ProQuest (Company) Dissertations & Theses Database (PQDT).
Submission of the thesis to the library requires the following:
___1. Two unbound copies of the thesis printed on archival quality paper (paper that is at least 25 percent cotton and acid free). The label on the box of paper may state acid-free or there may be a watermark on the paper indicating the same.
___2. One original, signed abstract with the first copy; a photocopy of the abstract is acceptable with the second copy.
___3. One original, signed approval page with the first copy; a photocopy of the approval page is acceptable with the second copy.
___4. If applying for copyright, a copyright page must be inserted immediately after the title page and the "Copyright Registration Form“must be signed. This form is on page 6 of UMI's "2008-2009 Master's Thesis Publishing Agreement" mentioned in step #6 below.
If NOT applying for copyright, a blank sheet of paper must be inserted immediately after the title page of dissertation.
___5. Two extra copies of the title page and one extra copy of the signed abstract. These do not have to be on archival quality paper.
___6. Completed and signed UMI's "2008-2009 Master's Thesis Publishing Agreement" Form. This multi-page agreement form is kept on file at Miller Nichol’s Library Reference Desk or obtain a copy from this website:
http://www.il.proquest.com/dissertationagree
Username: dissertations Password: publish
Click on: "2008-2009 Master's Thesis Publishing Agreement."
Choose one of the two publishing options for your thesis: either “traditional publishing” or “open access.” Traditional publishing allows UMI to microfilm your thesis and place the abstract in their database while open access allows for free global access to your entire thesis via the Internet. Open access is more expensive but has a much wider distribution. You may also choose restrictions which might apply to the publishing and distribution of your thesis. Read pages 3 and 4 of this agreement to determine your needs.
___7. Cashier's check or money order made out to: ProQuest Information and Learning Company. Note: No personal checks accepted! Please include your name and address on the cashier's check or money order.
Choose one of the following ProQuest charges for the publishing and processing of your thesis:
Traditional Publishing
- FILING for copyright: $120
- NOT filing for copyright: $55
Open Access Publishing
- Filing for copyright: $215
- NOT FILING for copyright: $150
___8. "Certification of Acceptance" form, from the School of Graduate Studies (you need an original plus two photocopies). All three copies will be signed and dated by the librarian on duty to indicate that all of the previous steps have been properly completed. Here is what happens to these three copies:
- The library sends the original "Certificate of Acceptance" to the registrar's office.
- You should give one copy to the chairperson of your committee.
- You will keep one copy, as a receipt that the thesis has been submitted successfully.
