Interlibrary Loan Request
UMKC faculty, staff and students may use this form to request materials not owned by the University Libraries.

Interlibrary Loan Renewal Requests Forms
Use these forms to request the renewal of items received through Interlibrary Loan. Interlibrary Loan items have purple or pink bookbands and an ILL number. Please, make your renewal request before the due date given on the band of the book. Renewal dates depend on the lending library's policy.

Reference Inquiry
Use this form to request reference assistance from any of the UMKC Libraries. It will help you with brief questions on factual matters and information about library holdings and policies.

Purchase Request
Faculty and staff are invited to request materials to be purchased for the UMKC University Libraries' collections. The order deadline for the current fiscal year is March 1, 2008.

Instruction Request
Library Instruction sessions are designed to introduce students to the research skills needed to complete a specific assignment as well as to provide the basic knowledge to recognize when information is needed, how to locate it, and how to use it. When the goals of a course involve library research, classroom faculty are encouraged to schedule a library instruction session. Use this form to submit your request for library instruction.

Reserve Request
To ensure the availability of course material to all students, UMKC faculty and staff may use this form to put items on reserve at the Miller Nichols Library.

Proxy Extend Form
A service the Miller Nichols Library offers to full-time faculty members is the establishment of a proxy library account. Full-time faculty members can use this form to enable their assistant(s) to check out materials from the Miller Nichols Library, make requests for interlibrary loan books and articles, and pickup interlibrary loan materials for them when they arrive.

Friends of the Library
Use this form to complete a membership application for the Friends of the Library, an organization dedicated to providing needed community support to strengthen collections and services of the University Libraries.
