Checklist for Doctoral Dissertations

Submission of dissertations requires the following:

• Two copies of the dissertation printed on archival quality paper (paper that is at least 25 percent cotton and acid free).
• The label on the box of paper may state acid-free or there may be a watermark on the paper indicating the same.
• If the paper is not marked as acid-free, a sheet of paper can be tested for acidity with the test pen at the Reference Desk.
• One original, signed abstract with the first copy; a photocopy of the abstract is acceptable with the second copy.
• One original, signed approval page with the first copy; a photocopy of the approval page is acceptable with the second copy.
• Two extra copies of the title page and one extra copy of the signed abstract. These do not have to be on archival quality paper.
• Completed and signed UMI Doctoral Dissertation Agreement Form (kept on file at Miller Nichols Library Reference Desk).
• "Certification of Acceptance" from the School of Graduate Studies (a 3-part carbon copy form or three photocopies) should be signed and dated by the librarian on duty.
• The library keeps the original "Certificate of Acceptance" with the dissertation. It will be mailed to the Registrar's Office.
• The remaining two copies of "Certificate of Acceptance" are returned to the student; one copy goes to the chairperson of his/her committee and one copy is kept by the student as a receipt that the dissertation has been submitted.
• If student has only the original "Certificate of Acceptance," this is still acceptable. Copies can be made and each of them signed.
• Cashier's check or money order (listing name and address of student) made out to ProQuest Information and Learning Company. (No personal checks accepted!)
• This charge is for publishing and processing by ProQuest.

NOT FILING for copyright: $70.00

FILING for copyright: $135.00 (Increase due to the Library of Congress & ProQuest charges.)

• If applying for copyright, an “Authorization to Apply for Registration of My Claim to Copyright,” form must be completed in addition to the usual Agreement form. This form may be picked up either at the Office of the School of Graduate Studies (SGS), or at the Miller Nichols Library Reference Desk.
  
  
• If applying for copyright, a second signature is required on the back of the UMI Doctoral Dissertation Agreement Form.


• If applying for copyright, a copyright page must be inserted immediately after the title page.


• If NOT applying for copyright, a blank sheet of paper must be inserted after the title page of the dissertation.
